Large-Volume Ultrasound-Assisted Lipoplasty

ABSTRACT
With the introduction of the tumescent technique, the removal of large volumes of fat during a single liposuction procedure has increasingly been reported. Ultrasound-assisted lipoplasty (UAL) offers a new approach to large-volume liposuction. The selectivity of ultrasonic energy for fat cells limits blood loss, effectively treats difficult fibrous regions of the body, provides excellent control in contouring, and minimizes surgeon fatigue. This report reviews patient selection, preoperative preparation, choice of anesthesia, fluid management, and lidocaine administration. The three stages of UAL are reviewed, including the infiltration of wetting solution, application of ultrasonic energy, and fatty emulsion evacuation. Special emphasis is placed on techniques to avoid complications in large-volume UAL.
