Ein Modell zur Qualitätssicherung bei Präventions-/ Rehabilitationsmaßnahmen am Beispiel der Kur für den alten Menschen - Visualisierung eines ganzheitlichen Ansatzes

Summary

Within the framework of preventive and rehabilitation measures on an inpatient basis a standardised “geroprophylactic screening” procedure is now well-tried in respect of everyday activities whose non-performance due to disability would seriously impair basic conditions of living. This screening procedure is also a suitable medium to assess the relevance of the many diagnoses made in persons of advanced age, in respect of their restrictive influence on mobility and functions and hence on the impending loss of autonomy. The screening comprises the following parameters.

Cardiopulmonary capacity, brain function status, analysis of competence and incompetence factors, psychosocial level, condition of joints and vertebral column, analysis of medication, analysis of risk factors including relevant laboratory data, clarification of problems specific to diagnosis and pertaining to the areas of internal medicine, orthopaedics, neurology, gynaecology and urology, in accordance with the multimorbidity associated jointly and severally with these factors.

The transfer of this “geroprophylactic screening” to a graphic method of visualisation condenses the large number of medical data into assessment scales (assessment curve: course of treatment - effect of treatment). This shows at a glance the degree of remaining patient autonomy or individual functional restrictions on which therapy will have to focus. The assessment criteria of medical expertise are suitable for determining the grade of incapacitation of the individual patient. These criteria have been officially laid down in Germany. The progress and efficacy of health cure treatment can thus be represented by the quality of the treatment procedure or, in other words, of the results achieved.

Zusammenfassung

Im Rahmen von stationären Präventions-/Rehabilitationsmaßnahmen hat sich ein standardisiertes „Geroprophylaxe-Screening” bewährt in den Funktionsbereichen, deren Einschränkung im Krankheitsfall die „Verrichtungen des Grundbedarfs” bedrohen. Es ist auch geeignet, die im Alter oft vielen Diagnosen bezüglich ihrer Relevanz auf die Funktionseinschränkungen zu übersetzen und sie in ihren Auswirkungen auf den drohenden Autonomieverlust zu bewerten:

Die„kardiopulmonale Belastbarkeit”, der „Hirnleistungsstatus”, die „Analyse der Kompetenz- und Inkompetenzfaktoren”, die „psychosoziale Ebene”, den „Gelenk- und Wirbelsäulenstatus”, die „Medikamentenanalyse”, die „Risikofaktorenanalyse einschließlich relevanter Laborbefunde”, die „Klärung diagnosespezifischer Fragestellung auf internistischem, orthopädischem, neurologischem, gynäkologischem und urologischem Gebiet", je nach Multimorbidität.

Die Übertragung dieses „Geroprophylaxe-Screening” auf ein grafisches Visualisierungsverfahren (Einschätzungsgraphik: Kurverlauf-Kureffekt) verdichtet die Vielzahl der medizinischen Daten in Bewertungsskalen. Auf einen Blick werden so die Intaktheit der Selbständigkeit bzw. der Grad der Autonomie oder das individuelle Ausmaß der einzelnen Funktionseinschränkungen sichtbar, auf die das Behandlungskonzept schwerpunktmäßig auszurichten ist. Zur Bewertung des Grades der Beeinträchtigungen sind die Beurteilungskriterien der Gutachtermedizin geeignet, wie sie in den „Anhaltspunkte für die ärztliche Gutachtertätigkeit im sozialen Entschädigungsrecht und nach dem Schwerbehindertengesetz” hinterlegt sind. Hiermit ist der Kurverlauf/-effekt auch im Sinne der Prozeß- bzw. Ergebnisqualität darstellbar.
