Low Dose of Losartan Decreased Urinary Excretions of IgG, Transferrin, and Ceruloplasmin without Reducing Albuminuria in Normoalbuminuric Type 2 Diabetic Patients

Introduction

For the early detection of kidney damage in diabetic nephropathy, several studies including ours have reported that urinary excretions of various plasma proteins, such as IgG [1] [2], transferrin (Tf) [3] [4], and ceruloplasmin (CRL) [2] [5] are increased in diabetic patients with normoalbuminuria (NA) and that such increases in the urinary protein excretions at baseline predict future development of microalbuminuria (MA) [4] [5]. Thus, to prevent the progression of MA from NA, it seems important to search a therapy by which increased urinary excretions of plasma proteins mentioned above would be decreased. Previously, we have reported that glycemic control decreases these urinary proteins without change in urinary albumin excretion (UAE) in diabetic patients with NA [2]. However, few studies have examined the effect of therapy other than glycemic control on these urinary protein excretions in diabetic patients with NA. Therefore, the present study was designed to clarify the effect of angiotensin II receptor inhibitor (losrtan) on these urinary protein excretions in normoalbuminuric diabetic patients.
