Intraduktaler papillär-muzinöser Tumor des Pankreaskopfes - Entität eines zystischen Pankreastumors

Zusammenfassung

Wir berichten über eine 55-jährige Patientin, die uns zur Abklärung eines Pankreaskopftumors zugewiesen wurde. Anamnestisch bestand eine chronische Pankreatitis seit 5 Jahren. Aufgrund abdomineller Beschwerden erhielt die Patientin zur Verlaufskontrolle eine Computertomographie des Abdomens, in der nunmehr ein Pankreaskopfprozess unklarer Dignität nachweisbar war. Aufgrund typischer ERCP- und Zytologie-Befunde wurde die Diagnose eines muzinösen Tumors des Pankreaskopfes gestellt und eine pyloruserhaltende Pankreaskopfresektion durchgeführt. Histologisch zeigte sich ein intraduktaler papillär-muzinöser Tumor des Pankreaskopfes vom Borderline-Typ. In der Kasuistik wird auf die Differenzialdiagnose zystischer Tumoren des Pankreas und deren unterschiedliche Therapie eingegangen.

Abstract

We report on a 55-year-old patient referred for diagnostic evaluation of a tumor of the head of pancreas. She had a history of chronical pancreatitis for 5 years. Due to abdominal pain a CT was made which revealed a pancreatic process of unknown origin. On the basis of characteristic ERCP and cytology findings the diagnosis of a mucinous tumor of the head of pancreas was made. The patient underwent a pylorus-preserving pancreaticoduodenectomy. The histological examination showed an intraductal papillary-mucinous tumor (borderline-type). Here we discuss the differential diagnosis of cystic neoplasms of the pancreas and their therapeutic strategies.
