PICA–PICA Reimplantation for the Treatment of a Partially Thrombosed Pica Aneurysm

This is a case of a 42-year-old male presenting with vertigo, headaches, and nausea. He was found to have a large, partially thrombosed right PICA aneurysm with an AICA–PICA anatomical variant. Tortuosity of PICA around the aneurysm neck precluded safe endovascular treatment and it was felt that the aneurysm could not be clipped. We demonstrate excision of the aneurysm from the circulation with PICA–PICA reimplantation via a far lateral, transcondylar approach.
