Zertifiziertes Qualitätsmanagement in einem telemedizinischen Schlaganfallnetzwerk

Zusammenfassung

Hintergrund: Die Versorgung von Patienten mir Schlaganfall hat sich aufgrund neuer Entwicklungen in der Akuttherapie in den letzten 20 Jahren grundlegend verändert. Um eine möglichst flächendeckende fachärztliche Versorgung sicherstellen zu können, haben sich in den Schlaganfallnetzwerke mit und ohne Telemedizin gebildet. Eine große Herausforderung stellt hierbei die Sicherstellung von einheitlichen Behandlungsstandards dar. Für die sehr erfolgreiche und sinnvolle Zertifizierung einzelner Stroke Units gab es im Bereich von Netzwerken kein entsprechendes Äquivalent. Wir berichten hier über die weltweit erste Implementierung und Zertifizierung eines netzwerkweiten Qualitätsmanagements. Methoden: Das Schlaganfallnetzwerk mit Telemedizin in Nordbayern (STENO) besteht aus 20 assoziierten Kliniken und ist mit über 5 000 Patienten pro Jahr eines der größten Schlaganfallnetzwerke weltweit. 2010 begann der Aufbau eines netzwerkweiten Qualitätsmanagementsystems nach DIN ISO 9001:2008 in Zusammenarbeit mit der Deutschen Schlaganfall-Gesellschaft und der Zertifizierungsgesellschaft LGA InterCert. Ergebnisse: Im März 2011 erfolgte die Zertifizierung nach DIN ISO 9001:2008 durch eine externe Zertifizierungsgesellschaft. Das Zertifikat wurde ohne Abweichung in den Überwachungsaudits gehalten. 2014 erfolge nach Ablauf des Zertifikats die Rezertifizierung. Neben anderen Erfolgen stieg die Thrombolyserate zwischen 2009 und 2012 um signifikante 56% an. Resümee: Ein netzwerkweites zertifiziertes Qualitätsmanagement in einem großen telemedizinischen Schlaganfallnetzwerk ist machbar und könnte medizinische Versorgungsstrukturen und damit die Thrombolyserate verbessern. Outcome-Studien, die eine konventionelle und eine telemedizinische Schlaganfallversorgung vergleichen erscheinen dringend notwendig.

Summary

Background: Stroke care networks with and without telemedicine have been established in several countries over the last decade to provide specialized stroke expertise to patients in rural areas. However, methods of standardization of care and treatment are much needed. To the best of our knowledge, we are the first stroke network to aim for certification of a network-wide quality management system. Methods: The Stroke Network Using Telemedicine in Northern Bavaria (STENO), currently with 20 associated medical institutions, is one of the world’s largest stroke networks, caring for over 5000 stroke patients each year. Results: Certification according to ISO 9001:2008 was awarded in 2011 and maintained over a complete certification cycle of 3 years without major deviation from the norm in three external third-party audits. Conclusion: Certified quality management within a large stroke network using telemedicine is possible and might improve stroke care procedures and thrombolysis rates.
