A New Kid in Town

Zusammenfassung

In der hier vorliegenden Kasuistik wird der Krankheits- und Behandlungsverlaufs eines 67-jährigen Patienten mit Morbus Hodgkin vorgestellt. Bei Erstellung der Erstdiagnose lag bereits ein Stadium IV B mit diffusem Knochenmarksbefall und multipler Lymphadenopathie sowie ossärer Metastasierung vor. Nach initialer Bestrahlung einer lokalisierten Osteolyse erhielt der Patient nachfolgend sequenziell zwei Chemotherapieprotokolle, auf die die Erkrankung jedesmal kurzfristig ansprach, aber dann immer schnell wieder progredient verlief. In dieser primär therapierefraktären Situation wurde dann das innovative Antikörperkonjugat Brentuximab Vedotin eingesetzt. Bei diesem zytostatisch wirkenden Medikament ist eine chemotherapeutische Komponente an einen monoklonalen Antikörper gekoppelt, der die Zielstruktur CD30 auf malignen Zellen detektiert.

Summary

The course of disease and treatment of a 67 year old patient with hodgkin’s disease is demonstrated with this case report. At time of diagnosis the disease presented already stadium IV with bone marrow infiltration, lymphogenic and bone metastasis. After an initial radiotherapy of a localized osteolytic process, the patient received to different sequentially applied chemotherapy protocols. The disease responded with some regression for short time, and then continued progressively. In this situation of primary refractory to conventional chemotherapy, a new and innovative target treatment, an anti-CD30 monoclonal antibody-drug conjugate that was approved in 2011 for the treatment of patients with anaplastic large cell and Hodgkin lymphomas, was introduced [2].
