Verletzungen der unteren Lendenwirbelsäule (LWK 3–5) entstehen typischerweise aufgrund von axialen Kompressionskräften. Im Vergleich zum thorakolumbalen Übergang entwickeln sich deutlich seltener kyphotische Fehlstellungen, sodass in der Regel eine konservative Therapie möglich ist. Neurologische Defizite und hoch instabile Frakturen stellen jedoch eindeutige Operationsindikationen dar. Die chirurgische Therapie wird im Sinne einer dorsalen transpedikulären Stabilisation mit einem Fixateur interne durchgeführt. Bei ausgeprägter Zerstörung des Wirbelkörpers erfolgt eine zusätzliche ventrale Stabilisation mit einem Wirbelkörperersatz.

Die klinischen Ergebnisse sind unabhängig von der Behandlung in der Regel gut bis sehr gut, auch wenn die Patienten häufig noch residuelle Rückenschmerzen angeben.
