Introduction

Urinary tract infection (UTI) is the most common bacterial infection of childhood (Koçak M, Pediatr Int 2016; 58: 467–471). Children with an obstructive urinary tract abnormality, such as ureterovesical junction obstruction (UVJO), are at increased risk of developing urinary tract infection. Infants and young children may have nonspecific symptoms such as fever, restlessness, refusal to feed and vomiting. In severe cases, symptoms and signs of urosepsis may develop, including impaired general condition and metabolic acidosis (Zorc JJ, Clin Microbiol Rev 2005; 8: 417–422).

Here, we report an infant with a diagnosis of obstructive uropathy who was thought to have developed sepsis secondary to UTI but was found to have an underlying metabolic disease.
