Introduction

Secondary metastatic disease to the ovary, including metastases arising from the gastrointestinal tract, breast, or female genital tract, is rare. An accurate diagnosis is challenging due to the wide range of differential diagnoses, especially in a patient without a prior known history of malignancy. We report a case of a 23-year-old woman with invasive, poorly differentiated small bowel adenocarcinoma metastatic to the ovaries, a rare tumor with a poor prognosis, who presented with worsening pelvic pain and symptomatic anemia.
