Ventrikuläre Arrhythmien bei Myokarditis und inflammatorischer Kardiomyopathie

Die sehr heterogenen Myokarditiden sind oft mit Unsicherheiten in Diagnose, Therapierbarkeit und Prognose verbunden. Gefährliche ventrikuläre Arrhythmien und der plötzliche Herztod können vorkommen und erfordern ein stringentes Vorgehen – häufig ohne gesicherte Diagnose. Dieser Beitrag zeigt, wann Gefährdungen entstehen und welche Risikobeurteilungen und Therapieoptionen empfehlenswert sind. Außerdem werden nicht ischämische Kardiomyopathien abgegrenzt.

Patienten mit akuter Myokarditis sollten je nach Risikofaktoren wie Nekrosemarkern, Herzinsuffizienzzeichen, Inflammationsnachweis und ventrikulären Arrhythmien stationär überwacht werden und eine strikte körperliche Schonung einhalten.

Myokarditispatienten mit bedrohlichen VTs oder drohendem Pumpversagen sollten an ein Maximalversorgungszentrum verlegt werden (Katheterablation, Kreislaufunterstützungssysteme).

Betablocker sind die erste Wahl der antiarrhythmischen Therapie bei Myokarditis. Als Additivum kommt Amiodaron infrage. Bei VTs aus der Bradykardie heraus muss ein passageres Pacing hinzugezogen werden. Klasse-I-Antiarrhythmika sind kontraindiziert.

Myokarditiden mit rhythmogenen Komplikationen und der Notwendigkeit einer zeitnahen spezifischen Therapie sind die Lyme-Karditis (AV-Blockierungen), die kardiale Sarkoidose (lebensbedrohliche VTs) und die Riesenzellmyokarditis (VTs und Leitungsblockierungen).

Bei chronischer Myokarditis kommen der Myokardbiopsie und dem LGE in der Kardio-MRT prognostische Bedeutung zu.

Die VES/VT-Ablation bei NICM sollte erfahrenen Zentren vorbehalten bleiben. Sie gilt als symptomatische Therapie, wenn Amiodaron nicht wirkt. Sie kann aber auch zur Vermeidung einer rhythmogenen Herzinsuffizienz prognoserelevant werden.

Die tragbare Defibrillatorweste ist durch hohe Effektivität und Sicherheit geeignet, temporär bei mitarbeitsfähigen Kardiomyopathiepatienten den plötzlichen Herztod zu verhindern.

Die NICM-Therapie sollte – vor Entscheidung über eine ICD-Implantation – bis zu 6 Monate lang optimiert werden. Bei Myokarditis-Risikoprädiktoren und insbesondere bei bereits therapiepflichtigen Kammerrhythmusstörungen scheint dabei ein temporärer Defibrillationsschutz mittels Defibrillatorweste angebracht zu sein.
