Akutbehandlung des Alkoholentzugs

Alkoholabhängige Patienten reagieren bei Ausbleiben der Alkoholzufuhr mit einer unterschiedlich schwer ausgeprägten Entzugssymptomatik. Diese erreicht meist 48 Stunden nach Abstinenz ihr Maximum. Verschiedene Komplikationen – allen voran das Delirium tremens – gefährden den Patienten während des Entzugs. Die Behandlung kann (teil-)stationär oder in geeigneten Fällen ambulant durchgeführt und medikamentös unterstützt werden.

Kernaussagen
  • Das akute Alkoholentzugssyndrom stellt eine Erkrankung dar, welche bei inadäquater Behandlung mit einer vitalen Gefährdung verbunden sein kann.

  • Das Delirium tremens ist eine der häufigsten Komplikationen im Rahmen eines Alkoholentzugs. Es sollte insbesondere bei vorausgegangenen generalisierten Krampfanfällen in Betracht gezogen werden.

  • Bei dem geringsten Verdacht auf eine Wernicke-Enzephalopathie sollte bereits zu Beginn eine hochdosierte parenterale Vitamin-B1-Therapie begonnen werden.
