Zusammenfassung

Hintergrund Nach Kleinschnitt-Kataraktchirurgie mit „nostitch”-Technik wird häufig postoperativ eine Drehung des induzierten cornealen Astigmatismus auf 90 Grad beobachtet, während nach „cross-stitch”-Technik oft ein höherer Astigmatismus mit der Regel festzustellen ist.

Patienten und Methode Wir haben eine konsekutive Serie von 426 Kataraktoperationen eines Operateurs ausgewertet, bei denen der Zugang mittels „frown incision” erfolgte. Der Wundverschluß wurde mit der „cross-stitch” (n=174), „one-stitch” (n=84) oder „no-stitch” Technik (n=168) durchgeführt. Den induzierten Astigmatismus bestimmten wir vektoranalytisch.

Ergebnisse Die Bedeutung der verschiedenen Einflußfaktoren wurde multivariat ermittelt und es zeigte sich, dass die postoperative Drehung der Achse des induzierten Astigmatismus wesentlich von der Schnittechnik und dem vorbestehenden Astigmatismus bei 90 Grad abhängt.

Summary

Background Small-incision cataract surgery has been further developed by the introduction of the sutureless technique. This procedure has reduced operation time and the surgically induced astigmatism compared to closure technique.

Patients and methods We evaluated 426 patients operated by the same surgeon using the same intraoperativ approach. Patients were monitored for postoperative induced astigmatism and postoperative complications.

Results Our resultats demonstrated small induced astigmatism after one week and one month. The no-stitch technique showed the smallest postoperative astigmatism with the least variation. With multivariat methods we also analysed several factors influencing the axis of the induced astigmatism.

Conclusion The most important factor for a postoperative astigmatism against the rule is using the no-stitch-technique and the existence of a preoperative astigmatism against the rule.
