Einleitung

Blepharitis und trockenes Auge

sind zwei unterschiedliche Erkrankungen; können beide gemeinsam auftreten; ihre Beschwerden ähneln sich; überlappen sich in ihren diagnostischen Schritten; müssen unterschieden werden, um eine angemessene Behandlung durchführen zu können.

Das trockene Auge (Tränenfilmdysfunktion): Ein trockenes Auge ist eine primäre Störung des Tränenfilms durch Tränenmangel oder exzessive Verdunstung, die zur Schädigung der Augenoberfläche führt und mit erheblichen subjektiven Beschwerden einhergeht. Schätzungsweise sind rund 10 Millionen Menschen in Deutschland von dieser Erkrankung betroffen; unter einer Beeinträchtigung im Alltagsleben und in der Freizeit leiden ca. 60 %, unter einer Minderung der Arbeitseffizienz etwa 38 % der Betroffenen. Zur Standarddiagnostik gehören die Messung der Tränenfilmmenge und ihrer Qualität. In Verbindung mit der Spaltlampenuntersuchung können zwei Formen des trockenen Auges unterschieden werden:

sekretorischer Tränenmangel, das sog. hypovolämische trockene Auge (Mangel an wässrig-muzinöser Sekretion), verstärkte Verdunstung und Störung des Tränenfilmaufbaues, das sog. hyperevaporative trockene Auge (mangelhafte Fettphase, Oberflächenproblem).

Behandlung: In der Regel wird derzeit der Mangel an Tränenflüssigkeit oder die mangelhafte Tränenfilmzusammensetzung durch Polymere kompensiert. Polymere sind Augentropfen oder Augengele, mit oder ohne Fettzusatz, die die kranke Augenoberfläche wieder glätten und benetzbar machen. Es gibt derzeit viele Produkte auf dem Markt, sodass für jeden Patienten eine individuelle Therapie zusammengestellt werden kann. Neuentwicklungen berücksichtigen die immunologisch-entzündliche Ursache des trockenen Auges und sollen auf molekularer Ebene eingreifen. Dazu gehören die bereits auf dem US-Markt erhältlichen Zyklosporin-Augentropfen oder die im Versuchsstadium befindlichen Androgen-Augentropfen oder Stoffe, die auf die Muzinsekretion Einfluss haben sollen.

Blepharitis: Rund 70 % aller Patienten mit Lidrandentzündung leiden gleichzeitig unter einer Hauterkrankung; die wichtigsten sind die seborrhoische Dermatitis, Akne, Rosazea und atopische Dermatitis (Neurodermitis). Weiterhin kommen als Ursache eine generell veränderte Fettsekretion oder lokalisierte Infektionen am Lidrand infrage. Die Diagnostik ist in der Regel überwiegend eine klinische „Blickdiagnostik“. Es werden folgende Formen der Lidrandentzündung unterschieden:

anteriore (vordere) Blepharitis betrifft vorzugsweise die Wimpernbasis: staphylokokkenbedingte Form seborrhoische Form posteriore (hintere) Blepharitis betrifft die Ausführungsgänge der Meibom-Drüsen.

Behandlung: Die Reinigung des Lidrandes ist Basistherapie!

lokale Antibiotika nur nach Keimnachweis bei staphylokokkenbedingter Form; Steroide nur kurzfristig und bei Hornhautbeteiligung, z. B. Akne rosacea; systemische Antibiotika zur Modulation des Meibom-Sekrets (Tetrazyklin und Derivate [Doxzyklin, Minozyklin] blockieren die bakterielle Lipase), Therapie mindestens 6 - 8 Wochen, manchmal bis zu 6 Monate; Tränenersatzmittel, ggf. mit Lipidzusatz.
