Summary
The filtration concept of the blood CSF barrier has become the basis for the interpretation of CSF data. It is now possible to quantify for every protein the serum derived fraction and that produced in the brain. Only the “intrathecal” fraction has diagnostic significance, e.g. that of the immunoglobulins G, A and M, the tumour proteins and antibodies. At present the research-oriented CSF analysis is looking for further cell-specific diagnostic markers for neurons (e.g. neuron-specific enolase), microglia (e.g. neopterin, ß2-microglobulin), choroid epithelium (e.g. transthyretin), Oligodendroglia (e.g. prostaglandin D synthetase) or endothelia (e.g. intercellular adhesion molecule).
Zusammenfassung
Das Filtrationskonzept hat sich für die Interpretation von Liquordaten als außerordentlich nützlich erwiesen. Mit seiner Hilfe ist es möglich, für jedes Protein die aus dem Serum durchgetretene und die im Gehirn synthetisierte Fraktion zu quantifizieren. Nur diese „intrathekalen” Fraktionen haben diagnostische Bedeutung, etwa bei den Immunglobulinen der Klassen G, A und M, den Tumorproteinen und den Antikörpern. Die heutige Liquoranalytik ist auf der Suche nach weiteren zellspezifischen Markern, etwa für die Nervenzellen (z.B. neuronenspezifische Enolase), Mikrogliazellen (z.B. Neopterin, ß2-Mikroglobulin), Plexusepithel (z.B. Transthyretin), Oligodendrogliazellen (z.B. Prostaglandin D-Synthetase) oder Endothelzellen (z.B. interzelluläres Adhäsionsmolekül).
