Zusammenfassung

Die Kenntnisse über den Einfluss von Weichteilen auf die Frakturheilung sind gering. Neue funktionell-anatomische Strukturanalysen und experimentelle Untersuchungen der Knochenbruchheilung weisen auf die Bedeutung des durch Weichteile induzierten Steuerungspotentials bei Wiederherstellungsprozessen nach Frakturen hin. Am Beispiel einer schwer dislozierten Aitken-I-Fraktur der Daumengrundphalanx im Rahmen einer komplexen Handverletzung bei einem 1,7 Jahre alten Jungen werden Möglichkeiten solcher Vorgänge erläutert. Innerhalb von 18 Monaten kam es zu einer spontanen kompletten strukturellen und funktionellen Ausheilung der Fraktur.

Abstract

The influence of soft tissue on bone remodelling after fracture is still poorly understood. Recent functional and anatomic studies indicate the significant role of soft tissue in controlling remodelling processes. The following report addresses this role. The potential of remodelling mechanisms is exemplified in a 1.7 year old boy with a severely dislocated epiphyseal fracture of the thumb. Within 18 months spontaneous and complete structural and functional healing was observed in this patient.
