Limitierte Resektion beim Barrett-Frühkarzinom

Zusammenfassung
Endoskopie mit bioptischer Diagnosesicherung sowie Endosonographie und CT erlauben verlässliche Aussagen zum Ausbreitungsgrad des Tumors als Entscheidungsgrundlage für die Therapieempfehlung. Da beim Barrett-Frühkarzinom vom Mukosa-Typ (pT1 m) das Risiko einer Lymphknotenmetastasierung nahezu null, auf jeden Fall aber geringer als das Operationsrisiko ist, tritt hier die organerhaltende endoskopische Mukosa-Ablation in Konkurrenz zur subtotalen transmediastinalen Ösophagektomie als chirurgische Standardtherapie. Beim submukösen Typ (pT1 sm) liegen dagegen in 18 - 50 % bereits Lymphknoten-Metastasen vor, so dass hier die Karzinombeseitigung nur chirurgisch gelingt. Allerdings sind überwiegend die in Tumornähe lokalisierten Lymphknoten befallen, deren Entfernung auch bei einer limitierten Resektion der distalen Speiseröhre und des proximalen Magenanteils mit nachfolgender Passagerekonstruktion durch ein Jejunuminterponat möglich ist. Diese von Merendino und Dillard [1] bereits 1955 zur Behandlung gutartiger Erkrankungen am ösophagogastralen Übergang inaugurierte, onkologisch vermutlich gleichwertige Alternative zur subtotalen Ösophagektomie reduziert das Operationstrauma und damit das perioperative Risiko, geht mit einer niedrigeren Morbidität als die ausgedehnteren Resektionsverfahren einher und liefert deutlich bessere funktionelle Ergebnisse bei gleichzeitiger Refluxverhütung.
Abstract
Endoscopy with biopsy, endoscopic ultrasonography and computed tomography (CT) are prerequisites for an exact preoperative staging of patients with esophageal cancer. Diagnosing an early adenocarcinoma limited to the mucosa (pT1 m), the risk of lymph node metastasis is nearly sero. Therefore, consideration could be given to ablate endoscopically the metaplastic mucosa including the pT1 m-area. On the other hand, an adenocarcinoma that is invading into the submucosa (pT1 sm) has an 18 - 50 % likelihood of associated lymph node metastases. In these patients with a highly curable form of esophageal cancer only esophageal resection with systematic abdominal and mediastinal lymph node dissection represents the standard of care. Because of substantial complications and long-term side effects the need for extensive resection in patients with early tumor stage (pT1) is questionable. Therefore, a limited resection of distal esophagus and proximal stomach with two-field lymphadenectomy and jejunal interposition is an attractive alternative in early Barrett's carcinoma providing a low morbidity and mortality and resulting in a 90 % chance of cure. The extent of esophageal resection should be tailored to the patient and the known extent of tumor disease.
