Koronare mikrovaskuläre Dysfunktion – 40 Jahre unterschätzt

Im Jahr 1985 wurde der Ausdruck „mikrovaskuläre Angina“ von Cannon und Epstein eingeführt für Patienten mit pektanginösen Beschwerden trotz unauffälliger Koronarangiografie [1]. In den letzten 20 Jahren konnten bezüglich der koronaren mikrovaskulären Dysfunktion (MVD) zwar diagnostische Erfolge erzielt werden, die Frage nach effektiven Therapiestrategien ist jedoch immer noch schwer zu beantworten.

Therapierefraktäre pektanginöse Beschwerden oder Dyspnoe sollten stets auch an das Vorhandensein einer mikrovaskulären Dysfunktion denken lassen.

Patienten mit mikrovaskulärer Dysfunktion haben einen hohen Leidensdruck und eine erhöhte Mortalität.

Hinweise auf eine mikrovaskuläre Dysfunktion ergeben sich aus pathologischen nicht invasiven Untersuchungen (Stressechokardiografie, PET, cMRT etc.).

Eine definitive Diagnosestellung ist invasiv möglich und sollte stets auch die Abklärung einer vasospastischen Komponente umfassen.

Moderne Therapieansätze der refraktären Angina pectoris bei mikrovaskulärer Dysfunktion werden aktuell im Rahmen von klinischen Studien erprobt.
