Welcome to the first issue of the new year 2024. This is the first issue of the seventh year of our journal. We wish you all a very peaceful year. We have a variety of articles in this issue that will interest you and help you focus your thoughts on your clinical practice.

Two closely linked comprehensive and practical review articles that considered the injectable therapies for diabetes and obesity depict the movement from the evolution of various insulin types to the revolutionary game-change brought about by the new twincretin, as highlighted by Khalil and colleagues in their commentary. The first review compares the insulin analogs and human insulins from various clinical and pharmacological aspects. In the second review, Wardeh et al review the evidence for the new dual GLP-1/GIP agonist tirzepatide and examines its place in the armamentarium of diabetes and obesity. A systematic review by Muammar and Alhasaeri analyzes the impact of growth hormone therapy on the final adult height of children with idiopathic short stature.

Two case reports are included in the current issue. The first case by Muammar and Helal presents the case of a patient with Rabson–Mendenhall syndrome with severe type A insulin resistance and discusses if the need to act should be faster than disease progression. The second case report by Al-Ghaithi et al describes emotional stress-induced thyroid storm, an extremely rare entity in clinical practice.

Finally, we present the free communications of the Fifth Libyan Diabetes Conference, held on December 28–30, 2023 in Misrata, Libya. The event was a great success despite the political divide in the country. We welcome all conference highlights and summaries of all good events in the region.

As I start my second term, I would like to thank all those who supported the journal over the last 3 years and call on all my colleagues to continue to support our mission to produce a high-quality international journal with a regional flavor. The regional niche of the journal addresses both the readers' needs to find relevant reading material readily and the authors' needs for a relevant platform for publication of their research findings and opinions.

I wish you a happy, prosperous, and peaceful new year!
