Lupusinhibitoren im Kindesalter

Zusammenfassung

Lupusinhibitoren (LI) manifestieren sich im Kindesalter anders als im Erwachsenenalter. Beim Erwachsenen steht die Thrombophilie im Vordergrund, beim Kind die Blutung. Mit beginnender Pubertät ist mit beiden Formen zu rechnen. Die auffälligsten Gerinnungsbefunde zeigen Kinder mit LI und Blutungen, d.h. neben erheblichen Phospholipidinhibitor-Wirkungen auf die Aktivitäten der Faktoren VIII, IX und v.a. XI und XII eine teilweise oder komplette Eliminierung des Prothrombins. Bei Thrombophilie ist das endogene Gerinnungssystem in gleicher Weise betroffen, bezüglich des Prothrombins fehlen ausreichende Daten. Bei symptomlosen Kindern findet man meist nur Ll-typische aPTT-Verlängerungen. Sie fallen in der Regel nur zufällig auf und zwar bevorzugt bei Kleinkindern und sind offensichtlich passagerer Natur.
