Kernaussagen

Die Zunahme der Alzheimererkrankung und anderer Formen der Neurodegeneration aufgrund der älter werdenden Gesellschaft stellt die Gesundheitssysteme der westlichen Industrienationen vor große Probleme.

Liquorbasierte neurochemische Demenzdiagnostik spielt eine zunehmend wichtige Rolle in der Früh- und Differenzialdiagnostik von Demenzerkrankungen.

Änderungen der Demenzbiomarker treten in einem sehr frühen Krankheitsstadium, Jahre oder möglicherweise Jahrzehnte vor Beginn der klinischen Symptome auf.

Aktuell werden die Liquorbiomarker Aβ1-42, Aβ1-42/Aβ1-40, Tau und Phospho-Tau in der Früh- und Differenzialdiagnostik von Demenzerkrankungen eingesetzt.

Bluttests werden als potenzielle Alternativen zu Liquortests intensiv untersucht, wenngleich gegenwärtig Sensitivität und Spezifität nicht zufriedenstellend sind.

Neue Labortechnologien (z. B. Multiplexing) ermöglichen sowohl eine bessere Handhabung der wertvollen Proben als auch eine Verbesserung der Qualitätskontrolle.

Eine korrekte und standardisierte präanalytische Probenbehandlung ist wesentlich für die Messung und Ergebnisinterpretation.

Die Verbesserung der Inter-Labor-Qualitätskontrolle in der NDD ist dringend erforderlich.
