Kasuistik interaktiv: Bewusstlos nach Koloskopie – Ein neurologischer Notfall?[1]

Zusammenfassung

Nach unkomplizierter ambulanter Koloskopie wird ein Mann bradykard und bewusstlos im Wartebereich einer internistischen Praxis aufgefunden. Nach einer halben Ampulle Atropin steigt die Herzfrequenz an, die Bewusstlosigkeit bleibt bestehen. Auch Flumazenil bringt keine Besserung. Die herbeigerufene Notärztin sieht sich mit einer Reihe neurologisch unspezifischer Symptome konfrontiert und arbeitet die wahrscheinlichen Differenzialdiagnosen ab. Erschwert wird der Einsatz durch die ländlichen Organisationsstrukturen. Der Zustand des Patienten verschlechtert sich während der Diagnostik rapide, er wird intubationspflichtig. Erst ein Telefonat bringt die richtige Diagnose und deckt eine (beinahe) fatale Verkettung von Missverständnissen auf.

Abstract

Following an uncomplicated outpatient colonoscopy, a patient was found unconscious with bradycardia in the waiting room of a practice of an internist. After a half an ampoule of atropine, the pulse quickened but the patient remained unconscious. Even flumazenil did not improve the situation. The responding emergency physician was confronted with numerous, unspecific neurological symptoms and considered various differential diagnoses. The situation was aggravated by the rural infrastructure. In the course of the clinical investigation, the patient’s condition deteriorated rapidly and intubation was necessary. The correct underlying diagnosis was finally made following a telephone call and disclosed an (almost) fatal chain of misunderstandings.
