Widespread dissemination of endoscopic sleeve gastroplasty (ESG) would benefit from: (i) further data on efficacy after more than 2 years, including the weight-independent benefits; (ii) deceleration of suture dehiscence; (iii) reduced procedure costs and/or reimbursement; and (iv) development of technologies that allow performance of ESG with a shorter learning curve. The novel Nitinotes device addresses point (iv), as the procedure is relatively automated and efficient to perform. Reasons for the reported relatively low weight loss could be that the suture is not always full thickness due to the reliance on suction. Additionally, as with the Apollo OverStitch, cheesewiring of the sutures occurs in the months post procedure. However, Nitinotes is an important development with respect to the dissemination of ESG, and, together with the addressing of points (i) to (iii), will allow the true potential of ESG to be realized.
