Auf Betalaktamantibiotika bei anamnestischer „Penicillinallergie“ verzichten?
allergy to penicillin – no beta-lactam antibiotics?Abstract
Drug allergies are frequently reported by patients undergoing pre-anesthesia evaluation, potentially resulting in a suboptimal treatment with alternative substances. While about 10% of patients report to be allergic to penicillin, less than 1% have a true IgE-mediated allergy. Multiple factors contribute to this large prevalence, such as misinterpretations of common gastrointestinal side effects as “allergic”, a history of penicillin treatment for infectious mononucleosis resulting in skin rashes, uncritical acceptance of the diagnosis by medical staff or neglect to the fact that most patients will lose their sensitivity after 10 years. In addition to this, cephalosporins are regularly withheld from patients with penicillin allergy due to an unwarranted fear of cross-reactivity. Therefore, a correct identification of those in which penicillins or cephalosporins are truly contraindicated is paramount to avoid an unnecessary use of antibiotics with a less favorable spectrum.
Die am häufigsten berichtete Betalaktamallergie ist die Penicillinallergie, die mehr als die Hälfte der im Prämedikationsgespräch erhobenen Antibiotikaallergien ausmacht. Tatsächliche, klinisch relevante IgE- oder T-Zell-vermittelte Reaktionen sind jedoch selten. Ungeachtet dessen erhalten Patienten mit dem Label der Penicillinallergie häufiger gar keine Antibiotikagabe trotz Indikation und auch häufiger Substanzklassen mit höherer Organtoxizität und breiterem Spektrum, die Resistenzbildungen begünstigen und selbst nicht unerhebliche Nebenwirkungen sowie ein intrinsisches Allergierisiko per se haben.

Viele Patienten tragen fälschlicherweise das Label einer „Penicillinallergie“ und erhalten dadurch eine nicht optimale antiinfektive Therapie.

Eine diagnostische Aufarbeitung kann sich in Abhängigkeit von der Planbarkeit und der Größe des Eingriffs lohnen.

Eine nachgewiesene Penicillinallergie schließt die Verwendung eines anderen Penicillins nicht zwingend aus.

Die Gabe von Cephalosporinen ab der 2. Generation kann unabhängig von einer Penicillinallergie erfolgen. Das Risiko einer allergischen Reaktion auf das Ersatzantibiotikum ist hier etwa gleich hoch wie das Risiko auf eine allergische Reaktion auf Cephalosporine ab der 2. Generation.
