Zusammenfassung

Hintergrund Gangtherapeutische Maßnahmen auf dem Laufband werden zunehmend durch den Einsatz technischer Hilfsmittel unterstützt. Hierbei kommen bei hohen Beeinträchtigungen der Gehfähigkeit mit geringer Restfunktion teilweise Robotiksysteme und Exoskelette zum Einsatz. Diese sind aufgrund der Investitionskosten jedoch wenig verbreitet. Die vorliegende Studie untersucht den Einsatz eines Expandersystems im gangtherapeutischen Setting und objektiviert den Einfluss verschiedener Zugrichtungen und Applikationsorte auf das Gangbild.

Methode Insgesamt wurden 24 asymptomatische Probanden auf einem dynamometrischen Laufband untersucht. Sie absolvierten 12 Versuche mit bilateralen Expanderzügen (von ventral, lateral und dorsal) an Ober- und Unterschenkel.

Ergebnisse Varianzanalytisch zeigten sich für die verschiedenen Testbedingungen signifikante Veränderungen in nahezu allen spatio-temporalen und dynamischen Gangparametern.

Schlussfolgerung Basierend auf den Ergebnissen kann davon ausgegangen werden, dass der Expanderzug einen relevanten Einfluss auf das Gangbild und die am Gang beteiligten neuromuskulären Funktionseinheiten ausübt. Die Art und Richtung der Beeinflussung des Gangbildes ist dabei vom Applikationsort der Manschetten und der wirkenden Zugrichtung abhängig.

Abstract

Background Gait therapeutic measures on the treadmill are increasingly supported by the use of technical aids. Robotic systems and exoskeletons are partly used for high impairments of gait performance with low remaining function. However, these are not widely used due to the investment costs. This study investigates the use of an expander system in the gait therapeutic setting and objectifies the influence of different directions of pull and application spot on the gait pattern.

Method A total of 24 asymptomatic subjects were examined on a dynamometric treadmill. They completed 12 trials with bilateral expander pulls (from ventral, lateral and dorsal) on the thigh and lower leg.

Results The variance analysis showed significant changes in almost all spatio-temporal and dynamic gait parameters for the different test conditions.

Conclusions Based on the results, it can be assumed that the expander pull has a relevant influence on the gait pattern and the neuromuscular functional units involved in the gait. Character and direction of the influence on the gait depends on the application spot of the cuffs and the impact of the direction of pull.
