Nierenersatztherapie und Ultrafiltrations-verfahren beim kardiorenalen Syndrom

ZUSAMMENFASSUNG

Eine renale Funktionseinschränkung liegt bei etwa 2 Dritteln der akut dekompensierten, herzinsuffizienten Patienten vor. Therapeutisch stehen neben konservativen Optionen auch interventionelle Verfahren zur Verfügung. Die konservativen Therapiemöglichkeiten der akut dekompensierten Herzinsuffizienz kommen gerade bei renaler Insuffizienz oftmals an ihre Grenzen, z. B. durch Entwicklung einer Diuretikaresistenz. In diesem Fall kommen interventionelle Verfahren zum Einsatz – entweder zur alleinigen Ultrafiltration bei isolierter Volumenüberladung oder in Form einer Nierenersatztherapie mittels Hämo- oder Peritonealdialyse. Bei der reinen Ultrafiltration zum Volumenentzug ist die Studienlage uneinheitlich, sodass ein Nutzen unter prognostischen Gesichtspunkten nicht eindeutig belegt ist. Bei zusätzlich bestehenden Zeichen von renal-metabolischen Entgleisungen ist die Dialysetherapie Mittel der Wahl. Hierbei ist besonders die Peritonealdialyse aufgrund der höheren hämodynamischen Stabilität, positiven kardialen Effekte und hohen Patientenautonomie als vorteilhaft hervorzuheben.
