Zusammenfassung

Die Feinnadelpunktion der Schilddrüse ist ein etabliertes Verfahren, um Schilddrüsenknoten artdiagnostisch einordnen zu können. Die FNP ist dabei ein wenig belastendes, nebenwirkungsarmes Verfahren, welches eine wichtige Säule in der Schilddrüsenknotendiagnostik darstellt. In der Hand des erfahrenen Punkteurs in Kombination mit einem versierten Pathologen ist die diagnostische Aussagekraft hoch, sodass mittels der FNP eine relevante Anzahl diagnostischer Schilddrüsenoperationen vermieden werden können.

In dieser Übersichtsarbeit wird auf die Anforderungen des Robert Koch-Instituts an die Hygiene bei der Feinnadelpunktion der Schilddrüse eingegangen. Sonografiegesteuerte effektive Punktionstechniken werden vorgestellt. Die Befundungskategorien des Pathologen und ihre Auswirkungen auf das klinische Vorgehen werden aufgezeigt. Fallbeispiele illustrieren die Korrelation der klinischen Befunde mit den zytologischen und den makroskopischen Befunden sowie den folgenden histologischen Präparaten.

Abstract

The fine needle biopsy of the thyroid gland is an established procedure to be able to classify thyroid gland nodules. The FNB is a low-invasive procedure with few side effects which is an important pillar in thyroid node diagnostics. In the hands of an experienced investigator combined with a competent pathologist, the diagnostic significance is high, so that a relevant number of diagnostic thyroid surgical interventions can be avoided by means of FNB.

This review discusses with the hygienic requirements of the Robert Koch-Institute. Sonography guided effective puncture techniques are presented. The findings’ categories of the pathologist and their effects on subsequent clinical procedure are explained. Case studies illustrate the correlation of clinical findings with cytological and subsequent histological preparations.
