Seit den 1980er-Jahren diskutieren Experten die Frage, ob Patienten mit einem nicht kleinzelligen Bronchialkarzinom (NSCLC) im Stadium T1N0 überhaupt von einer Lappenresektion profitieren oder ob eine sublobare Tumorentfernung ausreicht. Die Erkenntnis, dass sich NSCLC auch intraalveolär-aerogen – STAS (spread through air spaces) – ausbreiten können, hat die Debatte neu angeheizt. Die Kritiker der nicht anatomischen Operation weiten die Kontroverse aus. Auch andere Lokaltherapien müssten in Zeiten von STAS hinterfragt werden. David et al. diskutierten in einem Editorial die aktuelle Datenlage.
