Zusammenfassung

Sowohl in der präoperativen Visite als auch bei intraoperativ verdächtigen Zwischenfällen kann sich die Frage nach einer Abklärung auf maligne Hyperthermie ergeben. Heute stehen mit molekulargenetischen Methoden und mit der Muskelbiopsie und Kontrakturtest 2 grundsätzliche verschiedene Testmethoden zur Verfügung. Beide diagnostischen Methoden haben Vor- und Nachteile – und ergänzen sich.

Abstract

Testing for malignant hyperthermia (MH) susceptibility is usually performed either in relatives of MH susceptible persons or in patients with an MH suspicious clinical event. There are two diagnostic options: muscle biopsy with in-vitro contracture testing and molecular genetic diagnosis. Patients with familial MH mutations are usually tested genetically. A genetic screening can be performed in clinical events with a high pretest probability. MH susceptibility can be confirmed by presence of a diagnostic MH mutation. Absence of MH mutations cannot exclude MH. The only option to exclude MH susceptibility is contracture testing

  • Die häufigsten Indikationen für die Testung auf MH sind eine positive Familienanamnese oder ein MH-verdächtiges Anästhesieereignis.

  • Eine MH-Diagnose ist nicht nur für die getestete Person, sondern für die ganze Verwandtschaft relevant.

  • Diagnostisch stehen die Muskelbiopsie mit In-vitro-Kontrakturtest und eine molekulargenetische Testung zur Verfügung.

  • Das MH-Diagnostik-Zentrum entscheidet aufgrund der vorhandenen Unterlagen bzw. des Familienstammbaums, ob eine genetische Testung sinnvoll ist.

  • Kann eine MH-Mutation nachgewiesen werden, besteht eine Empfindlichkeit für MH.

  • Eine negative genetische Analyse kann eine MH-Empfindlichkeit nicht ausschließen.

  • Der Kontrakturtest ist die einzige Möglichkeit, eine MH-Empfindlichkeit auszuschließen. Auf eine negative genetische Analyse muss deshalb ein Kontrakturtest folgen.
