Klassifikation und Therapieempfehlung der osteoporotischen Wirbelkörperfraktur

Osteoporotische Frakturen von Brust- und Lendenwirbelsäule zählen zu den häufigsten geriatrischen Frakturen, führen bei den Betroffenen zu einer signifikanten Einschränkung der Mobilität und sind mit einer erheblichen Folgemorbidität und -mortalität verknüpft. Anhand des bildgebenden und klinischen Gesamtbildes ist die individuelle konservative oder operative Behandlung abzuleiten. Entscheidend für den weiterführenden Krankheitsverlauf ist die adäquate Therapie der Osteoporose.

Stellenwert

Aktuelle Metaanalysen klinischer Studien schlussfolgern, dass die Kyphoplastie ein sicheres Verfahren ist, um den lokalen Rückenschmerz von Patienten mit osteoporotischen Frakturen zu bessern [27] [28].

Kernaussagen
  • Osteoporotische Wirbelkörperfrakturen sind häufig und gehen mit einer erheblichen Einschränkung der Lebensqualität der Patienten einher.

  • Zur Basisdiagnostik der Osteoporose gehören Anamnese, klinische Untersuchung, DXA-Messung und die Abnahme ausgewählter Laborparameter.

  • Sowohl nach konservativer als auch nach operativer Behandlung einer osteoporotischen Fraktur sollte eine adäquate medikamentöse Therapie eingeleitet werden.

  • Die Alignmentveränderung im Sinne einer Kyphosierung durch osteoporotische Wirbelkörperfrakturen führt zur mechanischen Dekompensation.

  • Der OF-Score gibt eine hilfreiche Behandlungsempfehlung osteoporotischer Frakturen unter Berücksichtigung der Frakturmorphologie (OF-Klassifikation) und begleitender wichtiger klinischer Parameter.

  • Im Fall einer notwendigen Operation sind die Wiederherstellung der biomechanischen Lastverhältnisse und des Alignments die Schlüssel für ein verbessertes Outcome und reduzierte mechanische Komplikationen. Hierfür stehen folgende Verfahren mit steigendem chirurgischem Aufwand und Zugangsmorbidität zur Verfügung:

    • perkutane Augmentationen (Kyphoplastie),

    • perkutane Schrauben-Stab Konstrukte (mit oder ohne zementaugmentierte Schrauben, ggf. in Verbindung mit Kyphoplastie (Hybridkonstrukte),

    • offene dorsale Verfahren mit Korrekturen des Sagittalprofils (Osteotomien)
      oder aber

    • selten auch ventrodorsale Verfahren (Wirbelkörperersatz).

  • Nur die Kombination mit konservativen Therapien führt mittel- und langfristig zum Erfolg operativer Maßnahmen.
