Paraesophageal Hernia: An Avoidable Complication Following Surgery for Hiatus Hernia with Gastroesophageal Reflux

Abstract

Nissen funduplication and Boerema gastropexy are the most accepted methods for the treatment of gastroesophageal reflux in the pediatric age group. Whenever narrowing sutures are placed to approximate the diaphragmatic hiatal crura a rare complication - paraesophageal hernia - might ensue.

Two such cases are presented. In one case the occurrence of intestinal obstruction - idiopathic ileo-ileal intussusception occurring in early postoperative stage - possibly played a role in the production of the paraesophageal hernia by raising the intra-abdominal pressure. Episodes of crying, a common feature of pediatric patients, presumably lead to a similar result.

We believe that a good anti-reflux operative procedure should not include any approximation of the diaphragmatic crura using narrowing sutures through the muscular fibers. At most these might be placed only in the peritoneal layer.

Zusammenfassung

Die Fundoplicatio nach Nissen und die Gastropexie nach Boerema sind die im Kindesalter am häufigsten angewandten Methoden zur Behandlung des gastroösophagealen Refluxes. Wenn dabei Nähte gelegt wern mit dem Ziel, die Zwerchfellschenkel des Hiatusschlitzes einander zu nähern, kann als seltene Komplikation eine paraösophageale Hernie entstehen.

Zwei solche Fälle mit dieser Komplikation werden vorgestellt. Bei einem Fall mag das Auftreten einer ileo-ilealen Invagination in der frühen postoperativen Phase und die dadurch ausgelöste Steigerung des intraabdominellen Druckes eine Rolle bei der Entstehung der paraösophagealen Hernie gespielt haben. Schreiphasen können wahrscheinlich dasselbe bewirken.

Wir glauben, daß eine gute Anti-Refluxoperation nicht auch eine Annäherung der Hiatusschenkel durch Nähte beinhalten sollte, die die Muskelfasern fassen. Diese sollten nur durch das Peritoneum gelegt werden.
