Atypische Gelenksymptomatik als Erstsymptom einer Varizelleninfektion bei chronischer myeloischer Leukämie

Zusammenfassung
Ein 25jähriger Patient mit seit 4 Jahren bekannter chronischer myeloischer Leukämie (CML) in chronischer Phase erkrankte akut mit Fieber bis 39,5°C und stärksten Schmerzen in Schulter- und Hüftgelenken. Ein pathologischer Gelenkbefund wurde nicht erhoben. Nachdem eine symptomatische Therapie mit Indometacin eine kurzfristige Besserung der Beschwerden gebracht hatte, entwickelte sich innerhalb von 72 Stunden ein foudroyantes Krankheitsbild mit hohen Temperaturen sowie klinischen und klinisch-chemischen Zeichen einer schweren Verbrauchskoagulopathie. Kurz vor dem Tod traten herpetiforme Effloreszenzen an Kopf und Stamm auf. Bei der Autopsie konnte histologisch und immunhistochemisch eine Varizelleninfektion mit Hepatitis, Ösophagusbeteiligung und schwersten Einblutungen in innere Organe gesichert werden. Bei immunsupprimierten Patienten muß auch bei atypischen Prodromalsymptomen an eine Varizelleninfektion gedacht werden, damit rechtzeitig eine kausale Therapie mit Aciclovir und Hyperimmunglobulinen begonnen werden kann.
Abstract
A 25-year-old man, known to have chronic myeloid leukaemia for four years, acutely developed a fever of 39.5° and severe pain in the shoulder and hip joints. There was no evidence of joint disease. Treatment with indometacin briefly improved the symptoms, but within 72 hours the patient developed a fulminant illness with high fever and clinical as well as biochemical signs of a severe consumption coagulopathy. Herpetiform efflorescences appeared over the head and trunk shortly before death. At autopsy there was histological and immunohistochemical evidence of a varicella infection with hepatitis, oesophageal involvement and severe internal organ bleedings. In immune-compromised patients with atypical prodromal symptoms a varicella infection must be considered so that causal treatment with aciclovir and hyperimmune-globulin can be begun in time.
