Zusammenfassung
Die Kombination der queren Präputial-Insellappentechnik mit der glans-glandulären Tunnelierung zur Herstellung der »Neourethra« hat sich als einzeitige Operation mit geringer Komplikatlonsrate bewährt. Komplette Dissektion der Chorda ist Voraussetzung. Um die neue Urethra zu bilden, wird ein vaskularisierter gestielter Lappen des inneren dorsalen Vorhautblattes longltuidinal präpariert. Dieser Lappen wird dann über 12 Charr-Katheter zylinderförmig gerollt und durch den glandulären Tunnel zur Glansspltze gezogen. Dadurch wird der neugeschaffene Meatus an die Spitze der Glans gebracht. Die Erfahrungen mit dieser Technik erbrachten gute funktionelle und kosmetische Ergebnisse bei geringer Rezidiv- und Fistelrate.
Abstract
The combination of the transverse ventral preputial island flap with glans channel meatus to create a »neourethra« even in cases of posterior hypospadias allows an adequate reconstruction in a onstage procedure with a low complication rate.
Complete chordee release, confirmed by the artificial erection technique is necessary. To build the new urethra from a vascularized island flap with complete mobility of the pedicle, the abundant subcutaneous tissue of the dorsum of the penis, which is vascularized is dissected in a longitudinal fashion. It is rolled into a tube over a 12 F catheter; then a glans channel is created, and the distal portion of the graft is delivered through this channel. The edge of the graft is fixed to the glandular tissue in a circumferential manner after the proximal anastomosis is made. In this way the new meatus is placed at the tip of the glans. Use of this technique provides good functional and cosmetic results, and a low-fistula rate.
