Die hormonelle und chirurgische Beeinflussung des Lymphknotenstatus beim Prostatakarzinom

Zusammenfassung

In der Arbeit wird zur Problematik einer operativen oder hormonellen Beeinflußbarkeit der Lymphknotenmetastasen beim Prostatakarzinom Stellung genommen. Nachuntersuchungsergebnisse bei einer Patientenserie mit durchgeführter pelviner En-bloc-Lymphadenektomie zeigen zum Kontrolltermin keinen Anhalt für lymphogene Metastasen. Eine Patientengruppe mit kontrasexueller Hormontherapie weist in 20% der Fälle keine Metastasenremission und vereinzelt das Entstehen sekundärer Metastasen auf. Eine lokalisierte kurative Tumortherapie erfordert deshalb im individuellen Fall die operative Miteinbeziehung des regionären Lymphknotenstatus.

Abstract

The paper deals with the problems of operative and hormonal influence on the lymph node metastases in prostatic carcinoma. Follow-up results of a patient series with pelvic en bloc lymphadenectomy do not show any evidence of lymphogenous metastases at the control date. A patient group with contrasexual hormone therapy reveals no metastatic remissions in 20% of the cases and here and there the development of secondary metastases. Therefore, a localized curative tumour therapy requires the operative inclusion of the regional lymph node state in the individual case.
