Zusammenfassung
Bei einem 19jährigen Mann mit einer exzessiven renovaskulären Hypertonie wurde angiographisch eine ausgeprägte Stenose einer renalen Segmentarterie diagnostiziert. Nach Nephrektomie und Perfusion mit Euro-Collins-Lösung wurde die Gefäßstenose extrakorporal in Hypothermie mikrochirurgisch korrigiert und die Niere in die kontralaterale Fossa iliaca retransplantiert. Die Operation führte zur Normalisierung des Blutdruckes und signifikanten Verbesserung der Nierenfunktion. In ausgewählten Fällen stellt das extrakorporale Vorgehen ein bewährtes Verfahren zur Korrektur intrarenaler Gefäßstenosen dar.
Abstract
In a 19-year old male with excessive renovascular hypertension a pronounced stenosis of a renal segmental artery was diagnosed by angiography. After nephrectomy and perfusion with Euro-Collins solution the vascular stenosis was repaired microsurgically ex-situ in hypothermia, followed by autotransplantation of the kidney into the contralateral iliac fossa. The procedure led to normalisation of blood pressure and significant improvement of renal function.
In selected cases extracorporeal surgery represents a reliable method for the repair of intrarenal vascular stenoses.
