Die suprapubische Prostatasonographie zum Staging von Prostatakarzinomen

Zusammenfassung

61 Patienten mit histologisch gesicherten Prostatakarzinomen wurden mit der suprapubischen transvesikalen Prostatasonographie zur Frage des lokalen Tumorstaging untersucht. Die Ergebnisse wurden den rektalen Palpationsbefunden gegenübergestellt. Im Vergleich zur rektalen Palpation führte die Sonographie in 16 % zu einem Tumor-Upstaging und in 7 % zu einem -Downstaging. Die Untersuchungsergebnisse zeigten eine gute Übereinstimmung mit den Befunden nach Prostatektomie, Autopsie und Zystoskopie. Ergänzend wurden die sonographischen Upstaging-/Downstaging-Quoten mit den Literaturdaten der Tumorstadieneinteilung nach Prostatektomie im Vergleich zum klinischen Tumorstadium verglichen. Aus den Ergebnissen wird gefolgert, daß die suprapubische Prostatasonographie eine zuverlässige ergänzende Untersuchungsmethode für das lokale Tumorstaging ist und die lokale Tumorstadieneinteilung verbessert.

Summary

Sixty-one patients with histologically confirmed carcinomas of the prostate were examined by suprapubic, transvesical prostatic sonography in order to stage local tumour spread. The results were compared with rectal palpation. Compared with rectal palpation, sonography led to tomour up-staging in 16 % and down-staging in 7 %. The results showed good agreement with findings at prostatectomy, autopsy and cystoscopy. Additionally, the sonographie up-staging/ down-staging figures were compared with „tumour staging after prostatectomy, as compared with clinical staging” quoted in the literature. It is concluded that suprapubic prostatic sonography is a reliable supplementary method of examination for local tumour staging and that it improves the classification of local tumour extent.
