Rhabdomyosarkome sind selten vorkommende Tumoren der Prostata, welche nur schätzungsweise 0,1% der primären Prostatamalignome ausmachen. Grundsätzlich sollte ihre Behandlung in einem interdisziplinären Therapiemanagement erfolgen. Anhand eines eigenen Falles wird über das therapeutische Procedere und die zur Verfügung stehenden Therapiemodalitäten berichtet: Der jugendliche Patient präsentierte einen lokal weit fortgeschrittenen Tumor, ausgehend von der Prostata, ohne Hinweise für Lymphknoten- oder Fernmetastasen. Nach histologischer Sicherung wurde der Patient zunächst studienanalog zum CWS Protokoll 86 behandelt, wobei die initial verabreichte Chemotherapie nach dem VAIA-Schema nur einen mäßigen Tumorresponse zeigte und daher auf das PIAV-Schema umgestellt wurde. Nach abgeschlossener präoperativer Chemo-/Strahlentherapie wurde der Patient - unter Erhalt der Blase - radikal prostatovesikulektomiert und ist bislang, etwa viereinhalb Jahre nach Diagnosestellung, rezidivfrei. Ein interdisziplinäres Therapiemanagement bietet für Patienten mit Rhabdomyosarkomen des Urogenitaltraktes eine bessere Überlebenszeit und-qualität.
Abstract
Rhabdomyosarcoma of the prostate gland is rare, representing approximately only 0.1% of the primary neoplasias of the prostate gland. On principle, treatment should be interdisciplinary. An own case is presented and the therapeutical procedure and available treatment modalities are discussed: The young patient presented with local advanced tumor disease, but without lymphnode or organ metastasis. Following histopathological clarification, the patient underwent preoperative combined chemo- and radiotherapy analogous to the CWS-86 protocol. Due to insufficient tumor response according to the VAIA scheme, chemotherapy was changed to the PIAV scheme. After completing preoperative chemo/radiotherapy, the patient underwent radical prostatectomy without cystectomy and has been without evidence of disease up to now 4 years and 5 months after initial diagnosis. Interdisciplinary therapeutical management seems to ensure better survival rates and quality of life in patients with rhabdomyosarcoma of the urogenital system.
