Behandlungsergebnisse der habituellen Schulterluxation mit einer modifizierten OP-Technik

Zusammenfassung

Darlegung einer Modifikation der Original-Operationstechnik von M. Lange zur Behandlung der habituellen Schulterluxation, die im wesentlichen in einer Gelenkeröffnung mit Spaneinbringung unter visueller Kontrolle sowie Zuwendung der spongiösen Seite zum Humeruskopf und Kapselinterposition unter Raffung derselben besteht. Zusätzlich wird jedoch auch noch die Subskapularis-Raffung vorgenommen. Darlegung der Vorteile anhand einer Kasuistik von 57 Operationen, wovon etwa die Hälfte einer Nachuntersuchung mit mindestens 3jähriger, durchschnittlich 9jähriger Beobachtungszeit unterzogen wurde. In allen Fällen wurde eine Stabilisierung der Schulter mit sehr gutem bis gutem Ergebnis erzielt.

Abstract

The article describes a modification of the original surgical technique employed by M. Lange in the treatment of habitual luxation of the shoulder. This consists mainly of opening the joint with bone chip implantation under visual control, and turning the spongy side towards the head of the humerus and interpositioning the capsula while tightening the same. In addition, however, the point of insertion of the subscapular tendon is shifted while drawing it tighter also. The article describes the advantages, as seen from the case histories of 57 operations, about one-half being subjected to follow-up examinations over a period of at least 3 years, on the average 9 years. In all patients, excellent to good stabilisation of the shoulder was achieved.
