Zusammenfassung
Der Wert der Sonographie für die ätiologische Diagnose wurde bei 196 Patienten mit extrahepatischer Cholostase überprüft. Die prospektiv sonographisch erarbeitete Diagnose wurde mit der wahren Diagnose verglichen, die durch endoskopisch retrograde Cholangiopankreatikographie (ERCP), Computertomographie (CT), Operation oder Autopsie bzw. eindeutigen klinischen Verlauf gestellt wurde. Bei 72 Patienten war eine Cholangiolithiasis die Verschlußursache. Sie wurde mit einer Sensitivität von 86 % und einer Spezifität von 93 % sonographisch erkannt. Von 25 Patienten mit einer anderen benignen Ursache der Cholostase wurde die Diagnose in 23 Fällen durch die Sonographie richtig gestellt. Bei 93 Patienten wurde die zugrundeliegende maligne Erkrankung mit einer Sensitivität von 95 % und einer Spezifität von 100 % sonographisch diagnostiziert.
Summary
The value of sonography for aetiological diagnosis was examined in 196 patients with extrahepatic cholestasis. For each patient a diagnosis was prospectively worked out by sonography and compared with the true diagnosis established by endoscopic retrograde pancreatico-cholangiography (ERPC), and/or computed tomography (CT), surgical intervention, autopsy and unequivocal clinical course. In 72 patients cholangiolithiasis was the cause of the obstruction. It was diagnosed by sonography with a sensitivity of 86 % and a specificity of 93 %. With 25 patients with another benign cause of cholestasis the diagnosis was made by sonography in 23 cases. In 93 patients the underlying malignant disease was recognised sonographically with a sensitivity of 95 % and a specificity of 100 %.
