Diagnostische Wertigkeit der Angiodynographie bei pulsierenden Raumforderungen

Zusammenfassung
19 Patienten mit unklaren pulsierenden Raumforderungen nach Angiographie, Bypass-OP und Trauma wurden mit dem konventionellen Ultraschall, der Duplex-Sonographie, der Angiodynographie und teilweise Angiographie untersucht. Es fanden sich 9 Pseudoaneurysmen und 13 Hämatome, wobei sowohl mit der Duplex-Sonographie als auch mit der Angiodynographie die jeweilige Diagnose zu stellen war. Angiodynographisch waren beim Vorliegen eines Pseudoaneurysmas folgende typische Kriterien zu beobachten: 1. typische Wellenform in der Spektrumanalyse, 2. spezifisches Blutflußverhalten in den bzw. aus dem Aneurysmasack, 3. turbulenter Blutfluß im Aneurysmasack selbst. Insgesamt erlaubte die Angiodynographie eine sichere, schnelle und exakte Diagnose, so daß sie in Zukunft invasive aufwendigere diagnostische Verfahren wie die Angiographie ablösen könnte.
Summary
19 patients with undiagnosed pulsating masses occuring after angiography, by-pass surgery or trauma were examined by conventional ultrasound, duplex sonography, angiodynography and, in some cases, by angiography. There were nine pseudo-aneurysms and 13 haematomas; in each case the diagnosis was made by duplex sonography and by angiodynography. Pseudo-aneurysms show the following typical signs on angiodynography: 1. Typical wave form during spectrum analysis, 2. Specific blood-flow pattern into and out of the aneurysm, 3. Turbulence within the aneurysm. Angiodynography provides a reliable, rapid and exact means of diagnosis. In future, this will be able to replace more invasive procedures such as angioeraphy.
