Die Arthrodese des Großzehengrundgelenkes

Zusammenfassung

Seit der Erstbeschreibung durch Clutton 1894 sind zahlreiche Techniken der Großzehengrundgelenksarthrodese angegeben worden, die häufigsten Methoden werden beschrieben.

In der Klinik für Orthopädie der Medizinischen Hochschule Lübeck wird seit 1975 die Arthrodese dieses Gelenkes mit der Kleinfragmentplatte durchgeführt, Operationstechnik und Nachbehandlung werden skizziert.

Die Ergebnisse werden anhand einer Nachuntersuchung von 76 Patienten (95 Arthrodesen) beschrieben, es zeigt sich in Übereinstimmung mit den übrigen Literaturangaben die hohe Konsolidierungsquote von über 95%. Das poitive Urteil der operierten Patienten (80% sehr gut und gut) sowie die Endgültigkeit des Ergebnisses sind u. E. starke Argumente für eine weitere Verbreitung dieser Behandlungsmethode auch im europäischen Raum.

Abstract

Since it was first described by Clutton in 1894, numerous techniques for arthrodesis of the metatarsophalangeal joint of the great toe have been proposed; the most common methods are described.

At the Orthopedic Clinic of Lübeck Medical School, arthrodesis of this joint has been performed since 1975 using a small fragment plate. The surgical technique and postoperative management are outlined.

The results are described on the basis of follow-up examination of 76 patients (95 arthrodeses). They are in conformity with other figures cited in the litarature, with a high consolidation rate of over 95%. The positive rating by the patients thus treated (80% very good and good) and the definitive nature of the result are, in our opinion, strong arguments for adopting this method on a broader scale in Europe also.
