Akute multifokale Magenblutung bei Non-Hodgkin-Lymphom des oberen Digestionstraktes eines 7jährigen Mädchens*

Abstract

The acute gastric bleeding of children out of multifocal ulcers is an unusual leading symptom of systemic malignancy. Lymphomas of the gastrointestinal tract in children mostly are localized in the ileocoecal region and seldom in the stomach. The Zollinger-Ellison syndrom - only sporadically seen in childhood - is the first differential diagnosis. Therapy of acute ulcer bleeding consists of oversewing and medical prophylaxis of relapse and covers the directions of therapy of Zollinger-Ellison syndrom. Specific therapy of malignant lymphomas including polychemotherapy and radiation isn't possible before end of wound healing.

Zusammenfassung

Die akute Magenblutung des Kindes aus multifokalen Ulzera ist ein ungewöhnliches Leitsymptom einer malignen Systemerkrankung. Kindliche Lymphome des Gastrointestinaltraktes sind meist im Ileocoecalbereich und nur selten im Magen lokalisiert. Erste Differentialdiagnose ist das Zollinger-Ellison-Syndrom, das im Kindesalter ebenfalls nur vereinzelt beobachtet wird. Die Therapie akuter Ulkusblutungen besteht in Übernähung und medikamentöser Rezidivblutungsprophylaxe und schließt somit die Behandlungsrichtlinien des Zollinger-Ellison-Syndroms ein. Die gezielte Lymphomtherapie mit Polychemotherapie und Radiatio ist erst nach Abschluß der Wundheilung möglich.
