Zusammenfassung
Wir berichten über die Behandlung und den Verlauf von 6 Patienten mit einem unilateralen akuten akralen Ischämie-Syndrom. Es entsteht durch atheromatöse Mikroembolisation in die akrale Endstrombahn und erfordert wegen der schmerzhaften Hautnekrosen und der drohenden Zehengangrän ein dringliches Vorgehen. Bei allen Patienten führte eine Simpson-Atherektomie von proximal gelegenen femoro-poplitealen Stenosen zur Abheilung der prägangränösen Zehenveränderungen. Nach einer mittleren Beobachtungszeit von 10 Monaten trat kein Rezidiv auf. Als embolisierendes Material kommen parietale Fibrin-Plättchenthromben in Betracht, die angioskopisch sichtbar und histologisch regelmäßig im Stenosenmaterial nachweisbar waren.
Summary
We report on the treatment and follow-up of six patients with an unilateral „blue toe” syndrome. This is caused by atheromatous microembolisation to the digital arteries and requires urgent attention due to the painful cutaneous necroses and impending digital gangrene. In all patients, Simpson's atherectomy of proximally situated femoropopliteal stenoses caused the pregangrenous digital changes to heal completely. In a mean observation period of ten months no relapse occurred. The embolising material was presumably parietal fibrinoplatelet thrombi which could be observed with angioscopy and were regularly detectable histologically in the excised tissue.
