Zusammenfassung
Der diagnostische Stellenwert der Spongiosamorphologie für die Osteoporosediagnostik mit der quantitativen CT (QCT) wird dargestellt. Es können 5 verschiedene Spongiosamuster unterschieden werden, die mit der Spongiosadichte korrelieren. Diese Muster lassen allerdings nur in Einzelfällen osteoporotische Umbauvorgänge an konfluierenden Rarefizierungen erkennen, während die quantitativen Werte noch im unteren Normbereich liegen. Es besteht weder ein Vorhersagewert für das Ausmaß einer spontanen oder induzierten Abnahme der Spongiosadichte noch für das Ansprechen auf Therapie. Die Spongiosamuster in QCT–Bildern bleiben auch unter starken quantitativen Veränderungen der Spongiosadichte im kürzerfristigen Verlauf unverändert und sind daher ohne Aussagewert für Verlaufskontrollen.
Summary
The diagnostic ranking of trabecular morphology for the evaluation of osteoporosis in quantitative CT (QCT) is presented. 5 patterns of cancellous bone correlating with trabecular density may be discerned. However, these patterns may indicate osteoporotic changes in particular cases only, by confluent rarefications if the density is still within the normal range. There is neither a predictive value for the degree of spontaneous or therapy–induced bone loss, nor for therapy response. The trabecular appearances remain unchanged in short term even under strong quantitative changes and are therefore of no value for follow–up.
