Evidenzbasierte Empfehlungen der „European League against Rheumatism” zur Diagnose und Therapie der Hand-Arthrose

Zusammenfassung
Die Hand-Arthrose (hand osteoarthritis, im weiteren Dokument HOA) hat eine hohe Prävalenz und tritt häufig im Rahmen einer generalisierten Arthrose auf. Um Diagnose und Therapie für die tägliche Praxis gezielter gestalten zu können, wurde vom EULAR Standing Committee for International Clinical Studies Including Therapeutics (ESCISIT) eine Expertengruppe berufen, um Empfehlungen für die HOA zu erstellen. Im Rahmen eines Delphi-Prozesses wurden Expertenvorschläge ermittelt, ergänzend wurde eine systematische Literatursuche durchgeführt und schließlich ein Konsens erarbeitet. Die EULAR-Empfehlungen umfassen 10 Schlüsselaussagen zur Diagnose und 11 zur Therapie der HOA. Das typische klinische Bild einer HOA ist gut bekannt, jedoch gibt es wichtige Punkte, die bei der Diagnosestellung zu berücksichtigen sind. Dazu gehören Risikofaktoren, Demografie, klinische Manifestation, Verteilungsmuster und verschiedene Formen der HOA als auch die Kenntnis von radiologischen Veränderungen und der Wertigkeit von Laboruntersuchungen. Differenzialdiagnostische Schwierigkeiten werden durch die genaue Kenntnis der Klinik und Radiologie minimiert. Für die Therapie der HOA ist eine patientenorientierte Strategie notwendig, die pharmakologische Therapien mit ergotherapeutischen und physikalischen Maßnahmen, Schulung und orthopädischer Intervention kombiniert. Die Therapiemöglichkeiten, insbesondere Interventionen, sind nach Erkrankungsform und Lokalisation gesondert zu betrachten.
Abstract
Hand osteoarthritis (HOA) has a high prevalence and occurs either as a single entity or as part of a generalised osteoarthritis. To improve diagnosis and management of HOA in daily practice, the EULAR Standing Committee for International Clinical Studies Including Therapeutics (ESCISIT) has published recommendations which are based on expert opinions and a systematic literature search. The typical clinical signs and symptoms are well known; nevertheless, for accurate diagnosis further background knowledge is necessary. The 10 EULAR recommendations given for the diagnosis of HOA summarise risk factors, demographic aspects, signs and symptoms, subgroups, radiographic changes and laboratory tests as well as differential diagnosis. The management of HOA requires a patient-orientated strategy with a multidisciplinary approach. 11 EULAR recommendations focus on pharmacological therapies, occupational and physiotherapy as well as patient education and surgical intervention. Efficacy and safety of local steroids, paracetamol, non-steroidal anti-inflammatory drugs as well as symptomatic slow-acting drugs for osteoarthritis (SYSADOA) are discussed.
