The echographical investigation indicated by various accuses can detect some non symptomatical aspects, which need a careful differential diagnosis, evolutive supervision and those of the complications.

Case presentation: In a 16 years old teenager, the echographical exploration recommended for a urinary infection simptomatology resulted in detecting a large cyst formation –2,4/2,7cm – which was almost fully filling the right suprarenal gland, the left suprarenal gland being of normal size. The echography has permitted a differential diagnostic with other localized suprarenal diseases (benign or malign tumors, parasitic diseases, dysembriomas, tuberculosis, necrosis, hemorrhagical) or with surrounding structure's diseases (renal, hepatic, retroperitoneal, perirenal) The well limited formation aspect, with regular outline, clear liquid content, without the neighbouring invasion and without mass effect in the absence of clinical simptomatology as hypo or hyper corticoadrenal secretion has orientated for a simple solitary liquid cyst.

Conclusions:

1. The echography has permitted the rapid and non invasive detection of the suprarenal cyst, implying morphofunctional supervision especially within the growth period or within overburning periods with corticoadrenal stress.

2. The echographic aspect associated with other explorations (CT, biological tests) and also the clinical examination is pointing towards the differential diagnostic.

3. The echographical monitoring is necessary for evolution, possible complications and the eventual echographic guided treatment.
