Die Entwicklung einer gemeindebasierten Krankenversicherung - Erfahrungsbericht aus dem ländlichen Burkina Faso

Gemeindebasierte Krankenversicherungen sind ein neues und vielversprechendes Instrument zur Verbesserung der Krankenversorgung und zum Schutz vor mit Krankheit assoziierten finanziellen Härten in der benachteiligten ländlichen Bevölkerung Afrikas südlich der Sahara. Es gibt allerdings bisher nur wenig belastbare Daten zur Umsetzbarkeit und Wirksamkeit dieser Intervention. Die hier präsentierte Studie beschreibt die Erfahrungen bei der Entwicklung einer gemeindebasierten Krankenversicherung in Burkina Faso. Vorläufige Ergebnisse deuten darauf hin, dass im Versicherungssystem registrierte Haushalte die Nutzung der Gesundheitsdienste verdoppelt, dabei aber ihre Ausgaben für die Gesundheit verringert haben. Dies wäre ein erster Beweis dafür, dass die Etablierung von Krankenversicherungen im ländlichen Afrika südlich der Sahara einen positiven Effekt aufweist. Eine der größten Herausforderungen bleibt die weiterhin sehr niedrige Registrierungsrate von unter zehn Prozent der Haushalte der Studienregion.

In recent years, community health insurance has emerged as a promising health financing alternative, as it appears to facilitate access to care and offer protection against the cost of illness for traditionally disadvantaged populations in sub-Saharan Africa. However, the evidence needed to set up such insurance schemes and to evaluate their impact is still largely missing. This study attempts to fill this gap in knowledge, by following the development of a community health insurance scheme in rural Burkina Faso. Presently available results indicate that registered households have doubled their utilization of the available health services while reducing their own health expenditure. This may be considered as an initial proof of a positive effect of establishing a health insurance system in the rural sub-Saharan Africa. One of the greatest challenges is the still very low rate of registration from less than ten percent of the regional households in the area of this study.
