Chirurgische Intensivmedizin: Maßnahmen zur Qualitätssicherung und ein Überblick über die aktuellen Leitlinien

Ziele und Aufgaben der chirurgischen Intensivtherapie

Die chirurgische Intensivmedizin besitzt drei zentrale Aufgaben, nämlich die Reanimation, die Behandlung des Organversagens und schließlich die Prophylaxe von postoperativen bzw. posttraumatischen Komplikationen [41] [42].

Die Reanimation chirurgischer Patienten ist notfallmäßig in der überwiegenden Mehrzahl der Fälle im Rahmen eines akuten Herz-Kreislaufversagens erforderlich. Dieses kann zum einen durch primär kardiale oder respiratorischer Erkrankungen wie Herzinfarkt, Herzrhythmusstörungen oder Lungenembolie ausgelöst werden, zum anderen können aber auch spezifische chirurgische Komplikationen (z. B. Anastomoseninsuffizienz mit sekundärer Peritonitis und Sepsis) durch entsprechende Volumenverschiebungen zum akuten Herz-Kreislaufversagen mit konsekutiver Reanimationspflichtigkeit führen.

Die Behandlung des vital bedrohlichen Organversagens bildet quantitativ den Schwerpunkt der chirurgischen Intensivmedizin. Die Organdysfunktionen, die in der perioperativen Phase durch spezifische chirurgische Komplikationen, aber auch durch vorbestehende Risikofaktoren des Patienten ausgelöst werden, können zum klinisch manifesten Organversagen fortschreiten. Ab einem gewissen Schweregrad des Organversagens sind intensivmedizinische Maßnahmen zwingend erforderlich, um das Weiterleben des Patienten zu gewährleisten. Diese therapeutischen Maßnahmen haben weitgehend temporären Charakter und dienen dazu, Zeit zu gewinnen, bis die dem Mehrfachorganversagen zugrunde liegende Ursache therapiert ist. Somit ist es ein ganz entscheidendes Charakteristikum der operativen Intensivmedizin, sekundäre oder Folgeerkrankungen zu behandeln und nicht primäre Organdysfunktionen.

Schließlich stellt die chirurgische Intensivmedizin ein Instrumentarium dar, das nach chirurgischer Homoöstasestörung die gezielte Überwachung des Patienten erlaubt und somit allgemeinen Komplikationen sowie auch speziellen Organfunktionsstörungen vorbeugt.

Die chirurgische Intensivmedizin ist jedoch nicht in der Lage, chirurgische Fehler und Komplikationen zu beseitigen, zu diesem Zweck sind in der Regel nur chirurgische Maßnahmen geeignet. Deswegen stellt die chirurgische Intensivmedizin eine in anderen Bereichen der Intensivmedizin sonst nicht übliche Kombination aus den eigentlichen intensivmedizinischen Maßnahmen und den chirurgischen Interventionen dar. Operation und Intensivmedizin stehen somit gleichwertig nebeneinander, bedingen sich auch gegenseitig und müssen auch beide in die Qualitätssicherung mit einbezogen werden.
