Summary
Eighteen patients underwent combined preoperative irradiation and radical resection for a Pancoast tumor at the Department of Thoracic Surgery, Chest Disease Research Institute, Kyoto University between 1977 and 1993. Four patients were applied a full radiation dose of 50-70 Gy and fourteen patients were applied a reduced dose of 33-40 Gy preoperatively. Eleven of these fourteen were applied a supplemental dose postoperatively up to a total dose of at least 50 Gy. Fourteen lo-bectomies, three partial resections, and one pneumonectomy were performed with combined resection of chest wall or ad-jacent structures: rib in 14, vertebra in 4, brachiocephalic vein in 3, subclavian artery in 2, spinal nerve in 3, sympathetic truncus in 2, phrenic nerve in 2 cases. Chest walls were recon-structed with marlex mesh in 5 patients, and two subclavian arteries and one brachioeephalic vein were repaired with arti-ficial grafts. In 13 patients complete resections were achieved, but in the other 5 only incomplete resections leaving residual tumor were achieved. Incomplete resections consisted of 4 positive stumps at the brachial plexus of the apex and one aor-tic involvement by a metastatic lymph node. There was one operative death. Median survival was 21.6 months and the 5-year-survival rate was 38.5% for all 18 patients. In the complete resection group 5-year-survival was 56.4 %, but in the in-complete-resection group 0 %, showing a significantly more fa-vorable result for the complete-resection group. It is considered that evidence of incomplete resection influences the prognosis and that particularly tumor invasion to the brachial plexus may serve as a limiting factor for surgery.
Zwischen 1977 und 1993 behandelten wir 18 Patienten mit einem Pancoast-Tumor mittels einer Kombination von präoperativer und postoperativer Bestrahlung und anschließender radikaler Resektion. Bei 4 Patienten wurde bereits präoperativ mit der Höchstdosis von 50-70 Gy bestrahlt, 14 Patienten erhielten präoperativ lediglich 33-40 Gy. Von diesen 14 Patienten wurden 11 in der postoperativen Phase dann noch einmal bestrahlt, bis eine Gesamtdosis von 50 Gy pro Patient erreicht war. Der operative Eingriff bestand 14mal aus einer Lobektomie, 3mal aus einer atypischen Resektion und einmal aus einer Pneumonektomie, immer in Verbindung mit zusätzlichen Thoraxwandresektionen (14 Rippenresektionen, 4 Wirbelkörperresektionen, 3 Resektionen der V. brachiocephalica, 2 Resektionen der A. subclavia, 3 Spinalnervenresektionen, 2 Resektionen des N. phrenicus). Die Thoraxwand wurde bei 5 Patienten mit einem Marlexnetz rekonstruiert, zweimal wurde die A. subclavia und einmal die V. brachioeephalica mit einer Prothese ersetzt. Bei 13 Patienten konnten wir auf diese Weise eine komplette Resektion erreichen, 4 Patienten zeigten histo-logisch noch einen Tumorbefall der Absetzungsränder im Bereich des Plexus brachialis, ein Patient einen Lymphknoteneinbruch in die Aorta. Postoperativ verstarb ein Patient aufgrund einer bronchopulmonalen Fistel mit Empyem und Hemopty-sen. Die mittlere Überlebenszeit betrug 21,6 Monate und die 5-Jahres-Überlebensrate 38,5% für alle 18 Patienten. Im Falle einer kompletten Resektion betrug die 5-Jahres-Überlebensra-te sogar 56,4 %, dagegen war sie bei inkompletter Resektion mit 0 % signifikant niedriger. Wir folgern daraus, daß die Möglichkeit einer kompletten Resektion die Prognose entscheidend bestimmt, und daß der Tumorbefall dos Plexus brachialis einen limitierenden Faktor bei der Indikationsstellung zur Operation darstellen sollte.
