Zusammenfassung
Bei 50 Patienten mit 78 umschriebenen Lebertumoren bekannter Genese wurden farbdoppler-sonographische Untersuchungen durchgeführt. Die Parenchymdurchblutung wurde nach dem Ausmaß der “Tumoranfärbung” in 4 Typen unterteilt. In Lebermetastasen (n = 40) und Hämangiomen (n = 12) war farbdoppler-sonographisch keine relevante Durchblutung nachweisbar, während hepatozelluläre Leberkarzinome (n = 7) und die fokal noduläre Hyperplasie (FNH) (n = 16) überwiegend das Bild des hypervaskularisierten Tumors boten. Die Farbdopplersonographie kann somit in Ergänzung zur Sonomorphologie einen Beitrag zur Differentialdiagnose unklar umschriebener Lebertumoren leisten. Darüber hinaus liefert die Methode wichtige Informationen über die peritumorale Gefäßsituation und vaskuläre Komplikationen (Thrombose, Gefäßinfiltration).
Abstract
50 Patients with 78 focal liver lesions were examined via colour-coded Doppier system to study the vascularity of metastatic lesions and primary benign and malignant liver tumours. According to the amount of detectable colour Doppier signals, tumour vascularity was graded into 4 types. Metastases (n = 40) and cavernous haemangiomas (n = 12) seemed to be avascular with no evidence of increased vascularity (Type I/II). Conver-sely primary liver cancer (PLCA) (n = 7) and focal nodular hyperplasia (FNH) (n = 16) mainly produced arterial Doppier signals within the tumour (Type III/IV). AV shunts and partial portal vein thrombosis could be demonstrated in PLCA. Doppier colour flow imaging for detection of tumour vascularity may thus add to the specificity of ultrasonic tumour diagnosis.
